Registration Forms

I have a website for my Halo 2 Clan and i was curious how i could add a Registration Form to have it say their Rank on halo and their Gamer Tag, and so on and i was wondering if anyone knew how to make it so i could have it on my site and it gets sent to my email and i thought i should put it under HTML cause you have to code the Registration Form.

I think i could make the HTML part of it but I´m not so talented in PHP to code that part. Marc knows the PHP stuff, ask him.
