New Document

Make a new document, anysize doesn't matter. Press CNTRL+D to reset your pallete colors. Start off with any color of background, I used a dark grey.

Rectangular Marque Tool

Starting with the Rectangular Marque Tool now make a nice selection like below:

Right click the layer and apply these blending mode styles to it:

By having fill pixels selected this will save us from having to Rasterize the layer :D.

Make a White Box

Make a new layer and use the rectangular selection tool to make a white box like below:

Use the Rectangular Marque Tool

Now we want to make thin lines. Make 2 different colors, use the Rectangular Marque Tool to make these lines, or you can use the Line Tool with a 3 px width:

Note that I made the right line darker then the left line, you will see this in the next example pictures.

Blending Options

Use the Rectangular Marque Tool again to make yet another selection, remember you must make new layers everytime you make a new selection. Fill the selection in with your desired color and apply these blending options:

Make the same Colored Layer Again

Make the same colored layer but to the right, repeat step 5 to do this:

Press Delete

Select the white layer e created in step 3. Make a selection at the bottom like below and press delete!

Emerge Both the Layers

In step 4 we made those 2 lines. Emerge both the layers together. Now duplicate the layer and move them around. We are going to use them for content headers:

Add your Clipart and Icons

Now your pretty much done, I will leave you to completing the rest. When Designing a template its best to finish the blank design first then add your clipart and icons. Here is my final version with clipart and icons added:
